Contract of Residency Soul Survivor Supportive Living Foundation is entering this residency contract with Veteran/Civilian women. will be entering this agreement without dependents.This contract takes effect 20 and will automatically extend monthly unless a written termination agreement is submitted by either party.Soul Survivor Supportive Living Foundation Homes are a transitional living Home for women who are transitioning from military services and or societal barriers related to substance abuse, military sexually traumatized, incarceration, mental illness, and homelessness. Residents are subject to substance testing at any time while living at a Soul Survivor Supportive Living Foundation Homes. THE USE, AND/OR POSSESION OF DRUGS AND/OR ALCOHOL IS GROUNDS FOR IMMEDIATE EXPULSION. All policies and procedures outlined within the contract and any applicable subsequent amendments are in full force and effect during resident's entire residency at Soul Survivor Supportive Living Foundation Home unless specifically defined within a subsection of this contract. Violation of any policy or procedure outlined within this contract and any applicable subsequent amendments will probation/restrictions, and possible expulsion. Upon Arrival Residents will commence a 30-day probation period. Complete an Intake form and Consent Form. Read and sign House Rules Form. Read and sign House Contract. Pay a total of $1000.00 calculated as follows: $350.00 Administrative fees. $650.00 in Rent monies for the first Full Month of residency. Residents will be given a drug and alcohol test. II. During initial 30 days of Residency No visits with significant others and or family are permitted on premises unless prior approval is received from House Mother. Employment or school schedule must be obtained; and proof provided, within 30 days of initial date of residency at Soul Survivor Supportive Living Foundation House. Resident may not have any overnight stays off premises in the first 30 days. Curfew of 9:00 pm on weekdays and 12:00 am, on weekends. Resident must sign in and out when leaving or arriving at the House. At the completion of the initial 30 days of residency, a house vote will be taken, in concurrence with a consultation with the House Mother to determine the continuance of your residency. All residents must get approval from House Mother at least 24 hours prior to an overnight stay. III. Expense Responsibilities Rent is $650.00 per month. Groceries are not supplied by the house. Advance payments can be made for rent and/or other charges. There will be a $35.00 fee for returned checks. There will be a $35.00 late fee for rental payments made 24 hours after the 1st day of the month. Prorated Fees are as follows: $175.00 Per month for administrative fees (JUST FOR 2 MONTHS) $163.00(Weekly Rate) Tenants are responsible for the total cost of their damages at the Soul Survivor Homes. House Meetings Are Mandatory and Will Occur once a week from 7:00 pm until 8:00 pm. If you are unable to attend mandatory house meetings, you must receive prior approval from the House Mother. Employment Obligations Each resident must actively seek employment or registered in an educational program, full-time or part-time, and show proof monthly. If no proof of employment or registered in an educational program are provided there will be an immediate dismissal. Immediate dismissal will occur for the following reasons Disruptive behavior is defined as follows. ___ Violence or threats of physical violence. ___ Abusive verbal behavior. ___ Physical Violence. ___ A “chronic” bad attitude. ___ Gambling. ___ Destruction of house property. ___ Loud music, television, radio or instruments. ___ Theft. ___ Arson. ___ Breaking confidentiality. ___ Abusive/Loud arguments. ___Violation of house policy and procedures. ___ Use of drugs or alcohol on or off the property. ___ Possession and or use of weapons. ___ Refusal to provide a urine sample or alcohol test. ___ Not informing management when you know a resident is violating house rules. House Mother will make any decisions regarding expulsion and House rule changes. VII. Resident Responsibilities Residents must respect the anonymity of all residents. Resident and House business is confidential and must not be discussed outside of the house. Violation of the policy will be grounds for immediate expulsion. Residents will be responsible for completing chores. Chores will be assigned by House mother. Residents are expected to always maintain a respectful noise level while in the House. Rooms are subject to inspection at any time by House Mother. Rooms should be kept clean, and the beds should be made daily. On Saturdays, all House cleaning must be done by no later than 12:00 noon. House must be clean by all residents and inspected by the House Mother prior to anyone being able to leave premises. Residents are expected to plan their time accordingly to complete their individual assigned chores. All residents must volunteer 1 hour once a week with our partner organizations. VIII. Overnights Off Premises No overnight stays will be allowed during the initial 30-day probationary period. Overnight stays off premises must be requested through House Mother. Overnight stay requests must be received no later than 24 hours before the requested date. Overnight stays may not be accumulated. One overnight stay per week is allowed, unless preapproved by House Mother. Overnight stays are 24 hours, unless pre-approved by House Mother. IX. Medicine Residents must self-administer medication as prescribed by physician. Residents must not discontinue medication for any reason without a physician's order. Resident medication must be stored out of plain view from other Residents. For example, provided lockbox, in a dresser, closet, or in a container under the bed. Sharing medication is prohibited. X. Curfew House curfew Sunday through Thursday is 9:00pm. House curfew on Friday and Saturday is 12:00am. Exceptions will be made only for employment-related reasons (verification will be required). XI. Visitors All visitors must be pre-approved by House Mother. Visitors are allowed on the first floor only, but not in the bedrooms. Visitors will not be allowed to roam unattended in the house. XII. Telephone Free long distance and local calls can be made on the House phone; telephone curfew for incoming and outgoing calls is 9:00pm Sunday through Thursday. Calls are permitted until 12:00pm Friday and Saturday. Calls are limited to 15 minutes, when others are waiting. XIII. Smoking Smoking is prohibited in the house. Violation of this policy will result in possible dismissal. Smoking is permitted in designated areas only (Backyard). XIV. Termination of Residency A one-week notice is required prior to moving out. Upon leaving, Resident's living area should be thoroughly cleaned. Any Resident expelled for any reason will not be permitted to return to the premises as a Resident for a minimum of 30 days, and must go to a mission, a shelter, hospital, or detox. Expelled Resident must have a clean urine test in order to return. Resident will not be able to return to the premises as a Resident if they used any illegal drugs or alcohol prior to returning or possessed illegal drugs or alcohol on premises. Upon expulsion, personal belongings must be picked up within three (3) days or the belongings will be donated for charitable purposes. Prepaid rent monies will not be returned to Resident if asked to leave for a rule violation or relapse expulsion. This Contract and rules are subject to change without prior notice, by Board Members or House Mother at any time.
